By submitting this form, I am agreeing to the following terms and conditions of membership. Karate 4 Life is a brand of Sunshine Coast Karate, as such this agreement is between the student (or parent/guardian, if the student is under 18 years of age) and Sunshine Coast Karate. I understand the risk of studying martial arts and on behalf of myself, my heirs, executors and administrators, I hereby release Sunshine Coast Karate, all instructors and all other students of Sunshine Coast Karate from any and all liabilities of any nature (including any costs, whether not the subject of a court order), for any type of injuries or loss sustained while training, studying, practising or in the application of martial arts. I am in good physical condition and know of no reason why I cannot study and participate in martial arts or any activities conducted by Sunshine Coast Karate. • I agree to be bound by the rules of Sunshine Coast Karate. • I accept and understand the practice of martial arts involves a potential risk of serious injury • I understand that all fees are non-refundable (with a minimum 12 month training program) • Sunshine Coast Karate recommends you seek medical advice before beginning this or any other exercise program. • I understand that my tuition is arranged to be made by fortnightly/monthly instalments and is not affected by my lesson schedule and/or attendance. • I agree that Sunshine Coast Karate may take photographs or may make video and/or audio recordings of member's classes and school events, and that these materials may be used for display, promotion and/or advertising, or sold for profit, and the member hereby waives all rights to compensation which they may otherwise be entitle to for appearing in such material. • Hours of Operation – Times for classes may vary and are subject to change from time to time. Class schedules are displayed online. • Conduct during online classes – Management reserves the right to admission into classes, cancel a membership or request a member, parent guardian or guest of a member to leave the class if the person does not behave in a responsible manner, is under the influence of drugs / alcohol or does not adhere to the conditions of use. I agree to these terms and conditions of membership.
